This model is based on the so called “ early hospital discharge” application, that is to say a patient discharged in advance compared to the normal admission iter and continues to receive medical cares in one’s own house monitored from the same medical personnel and hospital that has followed the hospital stay from the beginning. It is like an “distributed hospital “ divided in a set of understructures constituted by the patients’ s houses, but all connected in a kind of star configuration, with at the middle the hospital and the medical personnel.
